  Care Planning

Introduction 

The operating framework for the NHS in England 2009/2010 states that 'everyone with a long term condition, (such as Diabetes), should be offered a personalised care plan'.  This is a plan that has been developed, agreed and reviewed with a named professional.

The care planning process should bring together all information about individuals into a single overarching plan.  The plan must include how diabetes impacts on their lives, with information to support self care so that everyone can make the best decisions about how to best treat and manage their condition.  This process leads on from DESMOND.

In Portsmouth and Sout East Hampshire we are taking a lead in implementing the stated objectives of care planning, and have produced a template for the care planning process which is in a developmental stage and awaits resourcing.  Please feel free to explore the documents and use them to help in the genesis of care planning.
