This site is a collaboration between the Diabetes Healthcare Professionals of Portsmouth and Hampshire who work in the community and the Diabetes team from Portsmouth Hospitals NHS Trust.

This website is intended as a resource both for healthcare professionals working in the area who need access to local contacts and pathway information, and also for individuals with diabetes who will want to be aware of the services available locally and how best to use them.
